In his "Gayatri-mahima-madhuri", Sri Mahanidhi Madana Gopal Dasa Babaji Maharaja writes the following: "The Bhaktavali says that a qualified guru should be pure in his dealings and saintly in character. He should be free from anger, pride and all material desires. He must be learned in Vedic shastras, speak sweetly, possess a handsome appearance, be full of compassion and do good to others. He should be peaceful, worship Radha and Krishna, be fully engaged in Their service and attached to chanting harinama."
After having searched for this "Bhaktavali," I did not manage to find any source of the book, either in Sanskrit or in English. Is it some Vaisnava-tantra unknown to the public? The only one source I am thinking about is found in Gopala Bhatta Goswami's "Hari Bhakti Vilasa" (I.38-41) in which there is a description of a guru's qualities ans is entitled "Mantra-muktavali":
मन्त्रमुक्तावल्याम्
अवदातान्वयः शुद्धः स्वोचिताचार-तत्-परः ।
आश्रमी क्रोध-रहितो वेदवित् सर्व-शास्त्रवित् ॥३८॥
श्रद्धावान् अनसूयश् च प्रिय-वाक् प्रिय-दर्शनः ।
शुचिः सुवेशस् तरुणः सर्व-भूत-हिते रतः ॥३९॥
धीमान् अनुद्धत-मतिः पूर्णो ऽहन्ता विमर्शकः ।
स-गुणो ऽर्चासु कृतधीः कृतज्ञः शिष्य-वत्सलः ॥४०॥
निग्रहानुग्रहे शक्तो होम-मन्त्र-परायणः ।
ऊहापोह-प्रकार-ज्ञः शुद्धात्मा यः कृपालयः ।
इत्य् आदि-लक्षणैर् युक्तो गुरुः स्याद् गरिमा-निधिः ॥४१॥
mantra-muktāvalyām—

ūhāpoha-prakāra-jñaḥ śuddhātmā yaḥ kṛpālayaḥ |
ity ādi-lakṣaṇair yukto guruḥ syād garimā-nidhiḥ ||41||
Some of these qualities correspond to what is stated in (or by) Bhaktavali, but not all of them, so my supposition is definitely worng, but I do not have better.
